Simple test that records user choices, gives pass or fail result at end of test

I see there have been other examples on the forum but they are all outdated. I would like to create a multi question test with a total of 5 questions. Each question has 4 answers to choose from with only 1 being the correct answer. Track results for each question, tally up results for 5 questions (must answer all questions correctly), at end you either pass or fail. Running on ls424 with a touchscreens. Has anyone created something like this that they can share? thanks.
